Note for providers, family members and others: We are not able to take referrals or submissions for another person (such as a patient or minor children (ages 0-17)) via this form. To make a referral or access services for a person you have legal guardianship over, please call (303) 504-7900 to speak directly with an intake specialist.
What to Expect From Your First Call
Whether you give us a call or request one from our team, your first conversation with WellPower will help us understand more about you and what's bringing you to WellPower. Staff will ask about the reasons you’re calling as well as some questions about your background, including whether you have the following information:
- Current and previous treatment information
- Insurance information, such as whether you have Medicaid, private insurance or are uninsured
- Social Security number
This will help determine if WellPower is a good fit. After that, our staff help identify which services you or your family member may need.
